৩.০ ওএমআর ফরম বিতরণ ও পূরন করা

ওএমআর ফরমের নির্দিষ্ট বক্সে বাংলায় ৩(তিন)টি বাক্য ও ইংরেজীতে ২(দুই)টি বাক্য আবশ্যিকভাবে লিখতে হবে।। রােল নম্বরের বৃত্তগুলাে ও সেট কোডের বৃত্ত ভরাট করতে ভুল করলে কিংবা হাজিরা শীটে (একটি নমুনা পরিশিষ্ট-৩ এ দেয়া আছে) স্বাক্ষর ও উপস্থিতি বৃত্তটি ভরাট না করলে তার পরীক্ষা বাতিল বলে গণ্য হবে। ওএমআর ফরমে সেট কোডের ঘরে প্রবেশপত্রে উল্লিখিত সেট কোডটির বিপরীতে বৃত্ত ভরাট করতে হবে।
৫.০ ওএমআর-এর সেট কোড ও প্রশ্নের সেট কোডের যােগসূত্রের ফরম যােগসূত্রের ফরমটি কি?
পরীক্ষার্থীদের প্রবেশ পত্র (একটি নমুনা পরিশিষ্ট-৪ এ দেয়া আছে)-এ একটি সেট কোড লেখা থাকবে এবং সেই সেট কোডটি ওএমআর ফর্মে পূরণ করবে হবে। যেমন- পদ্মা, মেঘনা, যমুনা……… ইত্যাদি। কিন্তু প্রশ্নের সেট কোড হবে ভিন্ন। যেমন- 1, 2, 3……….ইত্যাদি। কোন পরীক্ষার্থী কোন সেট কোডের প্রশ্নের উত্তর করবে তার একটি ম্যাপিং করা থাকে। যেমন- যে সকল পরীক্ষার্থীর প্রবেশপত্রে পদ্মা লেখা থাকবে তারা হয়তাে 2 সেট কোডের প্রশ্ন পাবে, যাদের প্রবেশ পত্রে মেঘনা লেখা থাকবে তারা হয়তাে 1 কোডের প্রশ্ন পাবে, ইত্যাদি। পরীক্ষা কক্ষের বাের্ডে আগে থেকেই ওএমআর-এর সেট কোড ও প্রশ্নের সেট কোডের যােগসূত্রের ফরমটি এঁকে রাখা হবে।
